In short: There is no legal BMI limit for IVF in the UK. Clinics and NHS funders set their own. NHS-funded IVF usually needs a BMI between 19 and 30. Private clinics most often use 35. At Avenues there’s no BMI cut-off: Fair IVF+ is IVF for people with a BMI over 35, at £4,749 all-in.
Is there a BMI limit for IVF in the UK?
Not in law. The Human Fertilisation and Embryology Authority (HFEA), which licenses UK fertility clinics, confirms there is no legal BMI limit. Each clinic, and each NHS funder, sets its own rules.
That’s why you can be told different things in different places. Avenues looked at 32 UK fertility providers in 2024. Their BMI limits ranged from 29.9 to 40. More than half (57%) used 35.
What BMI do you need for NHS IVF?
NHS IVF is funded locally by Integrated Care Boards (ICBs), and each ICB publishes its own eligibility criteria. In the same 2024 review, every ICB that published criteria (42 of them) asked for a BMI between 19 and 30, at referral or before treatment starts.
So if your BMI is over 30, you’ll usually be asked to lower it before NHS-funded IVF can begin. The exact rules, and whether there’s a waiting period, depend on where you live. Your GP or your local ICB’s fertility policy will have the details.
What about private clinics?
Private clinics set their own limits too, and they’re usually higher. The 2024 review found the average private limit was a BMI of 36, against 31 for NHS-funded patients, even within the same clinic. A BMI of 35 is the most common cut-off.
Does BMI affect IVF?
The verdict is still out. Research doesn’t agree on how much BMI affects IVF results, and there’s no clear BMI at which IVF stops working. NICE guidance says a higher BMI can mean it takes longer to conceive, and recommends weight loss for people who aren’t ovulating. It doesn’t say BMI should be a hard barrier to treatment.
Our own findings so far:
- Egg quality: our lab used an AI tool to score 1,031 mature eggs from 175 patients. The average egg-quality scores were almost identical across BMI groups, from under 25 to 40 and over, and BMI explained less than 1% of the differences.
- Egg collection safety: over a year, 56 patients with a BMI of 30 or more had egg collection on our nurse-led pathway. Every collection was completed, and no one needed a general anaesthetic or an unplanned hospital stay.
These are early results from our own clinic, and larger studies are needed to confirm them. Read more about our research.
Your doctor will look at your whole picture: your hormones, your scan, your medical history and your metabolic health, not one number.

Your questions, answered
What is the BMI limit for IVF on the NHS?
Most NHS funders ask for a BMI between 19 and 30. The exact criteria depend on your local Integrated Care Board.
Can I have IVF with a BMI over 35?
Yes. There’s no legal BMI limit in the UK, and some private clinics, including Avenues, don’t use a BMI cut-off. Avenues’ Fair IVF+ package is for people with a BMI over 35.
Can I have IVF with a BMI over 40?
At Avenues there’s no upper BMI limit. Your consultant will review your medical history and plan your care, including anaesthesia, around you.
Is BMI the only thing that decides if I can have IVF?
No. Age, egg reserve (AMH), your uterus, your partner’s sperm and your medical history all matter. At Avenues, eligibility is decided by your consultant after a full medical review.
Do I have to lose weight before IVF?
For NHS-funded IVF, usually yes, if your BMI is over 30. At Avenues it isn’t required. Your doctor will talk through what could help your chances, and the choice is yours.
